

Mental Health Care is a registered charity operating regionally in the North East of England. We have over 20 years experience in delivering high quality, specialist mental health services to vulnerable adults and older adults.
We have formed close working relationships with partners from Health and Social Care as well as other voluntary organisations and actively participate in provider forums.
Working collaboratively with the Primary Care Trust and Local Authority we are involved in the development of local and regional policy, commissioning strategies and services planning.
In our commitment to inclusion and service-user involvement, we support our clients to engage in consultation on a regular basis.
Each individual’s care is enhanced through the strong working relationships between our service staff and other professionals involved in the client's care pathway.
Mental Health Care works in partnership with Park House Surgery in Lanchester to enable provision of a dedicated GP service for all clients at Derwentside Dementia Care Centre. The GP and senior nurse practitioner both attend the service twice weekly to assess clients’ health, make recommendations and prescribe medication when needed. The GP and/or nurse practitioner attend the centre’s multidisciplinary meeting on a weekly basis to discuss and plan clients’ care. The dedicated GP service ensures frequent and regular assessment of all clients and, where required, family members are able to access time with the GP to discuss the needs of their relative.

Mental Health Care has established a unique working partnership with Macmillan Cancer Support. This involves a grant from Macmillan that is funding two posts to lead on a project focusing on cancer care for adults and older people with mental health conditions within mental health nursing and care settings.
The service model will involve implementation of the Gold Standards Framework and Advance Care Planning together with input from Macmillan, a local GP practice, and local specialist palliative care professionals.
We are committed to improving end of life care within our services and to raising awareness more widely.
Part of the roles of the Macmillan funded posts will be to collate data derived from this project for wider dissemination.
The Macmillan funded posts will also be central to skilling the workforce, sharing best practice across services and sectors, and raising the profile of the needs of this niche client group.
Mental Health Care is proud to have been the founding member of MHNE having secured the project’s first source of funding from The Northern Rock Foundation.
