Referrals
Referral enquiries can be made by telephone or by clicking on the link below and completing a referral enquiry form.
We will respond to enquiries within 48 hours and endeavour to book an assessment or client visit.
New referrals are able to visit the unit while waiting for funding to be arranged (upon receipt of a satisfactory risk assessment).
There is a 6 week assessment period for all new referrals once they join Derwentside Dementia Care Centre to ensure the centre can meet their needs.
We work in close partnership with clients’ relatives and carers and colleagues from Health, Social Care and other voluntary organisations to facilitate comprehensive care packages for our clients.
Derwentside Dementia Care Centre is a service newly registered with the Care Quality Commission. The service has been developed through a merging of two established Mental Health Care Services both of which have consistently performed well under CQC regulation.
The Specialist nursing unit is staffed at all times by qualified mental health nurses along with support staff.
The residential dementia unit is staffed at all times by senior care and care assistants.
Catering and domestic staff provide valuable services throughout the service.
The service provides placements to student nurses studying at Teesside University and is recognised as being a valuable placement opportunity.
